The University of Kansas Health System's Health Information Management (HIM) team is committed to protecting the privacy and security of your health information. Our team also ensures your medical records are available for continuing care, payment, personal health needs or other uses.
For patients: How to request your records
Request records for personal use or continuing care
Patients who need to access their records for personal use or continuing care can easily access or request records via MyChart, our dedicated patient portal. MyChart allows you to download, save, print or request copies of self-downloadable health records, including:
- Test results
- Immunization records
- Medication lists and costs
- After-visit summaries and clinical notes
How to print or save records in MyChart
- In the MyChart web browser, navigate to your desired visit summary, medication list, immunization record or other downloadable health record.
- In the upper right corner of the page, you’ll see a printer icon, a gear icon or 3-dot icon, depending on the web browser you choose.
- Click the icon and follow the prompts to print or save your record as a PDF.
How to request additional records
To request records that cannot be self-downloaded, log in to MyChart and select Request Medical Record on the main MyChart menu. Fill out the records request form completely and follow the prompts to submit the request.
You can also request your records through the Sharing Hub in MyChart. Click Sharing Hub from the main menu, choose Request Formal Copy of Health Record, then follow the prompts to request your records.

You can also request medical records for personal use or continuing care by completing our Patient-Directed Request form and returning it to Health Information Management.
Request medical records for deceased patients
To obtain a copy of a deceased patient's medical record:
- The executor or administrator of the estate must complete, date, sign and return the Patient-Directed Request form. A copy of the will or court order must accompany the completed form. A copy of the death certificate also must accompany the form, unless the patient passed at The University of Kansas Hospital.
- If there is no executor or administrator of the estate named for the deceased patient and the purpose of the request is to settle personal or financial affairs, complete the Access to Medical or Financial Records for Deceased Patient with No Executor form. A copy of the death certificate must accompany this authorization form unless the patient passed at The University of Kansas Hospital.
If you are requesting a deceased patient's records for purposes other than settling personal or financial affairs of the decedent, contact Health Information Management, 913-588-2454, option 1, or ROI@kumc.edu.
Request records for a 3rd party
If you need to send your medical records to a 3rd party, such as an insurance company or attorney, please complete the Patient-Directed Request form and return it to Health Information Management. These requests cannot be completed through MyChart.
Return the signed form via email to ROI@kumc.edu or fax 913-588-2495. You can also mail the signed form to:

For providers, 3rd parties and other stakeholders
The University of Kansas Health System provides support for all healthcare stakeholders to access electronic health information.
Healthcare providers
Physicians, advanced practice providers and other authorized healthcare providers can access patient records at The University of Kansas Health System in several ways:
- The health system participates in the LACIE and KHIN health information exchanges and Epic's Care Everywhere.
- EpicCare Link allows real-time access to the health system's electronic medical record for shared patients. Advanced signup is required. Contact Physician Relations for more information.
- Fax a request on office letterhead to 913-588-2495. Include:
- Patient's name
- Date of birth
- Fax number
- Type of information needed
- For after-hour emergencies, call 913-588-2454, option 1.
3rd-party requests for medical records, billing records or images
If you are a 3rd party, fill out and return the Authorization for Release of Confidential Information form. Send the completed form to:
- Return signed form via email to ROI@kumc.edu or fax 913-588-2495.

Subpoena and law enforcement requests
Attorneys and law enforcement officials seeking medical records should download, complete and return the Authorization for Release of Confidential Information form to:
