Clinic Services Authorization

Employee clinic service authorizations can be emailed directly to the clinics or submitted online.  Please include company name, employee name and services required. Please be specific on required services if needing a:

• DOT Physical/DOT Drug Screen
• Non-DOT Physical/Non-DOT Drug Screen
• Post Accident Drug/Alcohol Testing.

The authorization form is all that is required, and employees will only need to bring a form of identification.
